Court Prep Questionnaire

This questionnaire is to identify both:

1. Witnesses that you intend to call to testify on your behalf 
2. Exhibits - any document that you intend to use at trial  

This is to help me strategize for the upcoming hearing. 

For witnesses, no written statements will be accepted. If they wish to testify on your behalf, they must come in-person. Also, they can only testify to things that they have personally seen or heard directly. 

Please be sure to tell them that there is a chance that we run out of time and aren't able to call them. Once I receive the questionnaire, we'll discuss more as to whether we will be calling all of them or whether there will be some that we ultimately will decide to not call.
